Summary
The purpose of the research is to evaluate the feasibility and benefits of a six-week hybrid task-oriented resistance exercise program for older adults who have been recently discharged from home health care.

| BEHAVIORAL | Task-oriented resistance exercise | The intervention program includes progressive resistance exercise and daily activity exercise, delivered over a six-week period with an ideal frequency of three sessions per week. Progressive resistance exercise will be provided through a combination of in-person and online sessions, while daily activity exercise will be delivered exclusively in person. |
